Job summary

Crossroads Treatment Centers is seeking a Care Coordinator to meet with patients, coordinate recovery services, and document all activities in the patient record. The role requires collaboration with a buprenorphine‑waivered practitioner and local referrals to fill service gaps.

You will conduct monthly care coordination meetings to ensure ongoing engagement and effective support for individuals with substance use disorders.

Qualifications

  • Bachelor's degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling with 1 year of related experience or clinical experience with cooccurring disorders.
  • Registered Nurse license with 1 year of related experience or clinical experience with cooccurring disorders.
  • Board of Counseling CSAC or CSAC-Assistant under supervision as defined in regulations.

Responsibilities

  • Meet face-to-face with all patients to assist in facilitating recovery services and document activities in the patient record.
  • Utilize collateral contacts and local referral agencies to provide services not offered by the program.
  • Conduct monthly Care Coordination meetings with patients and document accordingly to ensure engagement.
  • Collaborate with buprenorphine‑waivered practitioner prescribing buprenorphine or naltrexone for OUD patients.
  • Perform other duties as assigned by the Operations Director of OBOT Services.

_ Day in the Life of a Care Coordinator _
  • Meet face‑to‑face with all patients to assist in the facilitation of recovery services and ensure that all recovery activities are properly documented in the patient's record

  • Utilize collateral contacts and other local referral agencies to provide services not offered by the program

  • Conduct monthly Care Coordination meetings with patients, and complete corresponding documentation, to assure that patients are engaged with the services they need

  • Working in collaboration with the buprenorphine‑waivered practitioner who is prescribing buprenorphine or naltrexone products to individuals with moderate to severe Opioid Use Disorder

  • Perform other duties as assigned by the Operations Director of OBOT Services

_ Education and Experience requirements _
  • A bachelor's degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling and at least either one year of experience providing services to individuals with a diagnosis of substance use disorders or a minimum of one year of clinical experience working with individuals with cooccurring diagnoses of substance use disorders and mental illness.

  • Licensure as a Registered Nurse with at least either one year of direct experience providing services to individuals diagnosed with a substance use disorder or a minimum of one year of clinical experience working with individuals with cooccurring diagnoses of substance use disorders and mental illness.

  • Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC‑Assistant under supervision of a CSAC or another supervisor as defined in 18 VAC 115‑50‑10
