Job summary

Qualifications

Responsibilities

• Provides continuity of care by ensuring smooth transitions between care settings.
• Develops relationships with patients and multidisciplinary team to facilitate and navigate treatment and follow-up.
• Manages high risk, complex patient care to minimize readmission.
• Coordinates care plan communication with physicians and multidisciplinary team; advocates for patient.
• Prepares, executes, and reinforces post-discharge care plan.
