Job summary

Qualifications

• Graduate of an accredited school of nursing.
• RN license in state of practice, BLS preferred.

Responsibilities

• Provides continuity of care by ensuring smooth transitions between care settings.
• Develops relationships with patients and the multidisciplinary team to facilitate post-discharge follow-up.
• Manages high-risk, complex patient care to minimize readmission.
• Coordinates with physicians and team to maintain up-to-date care plans.
• Prepares and reinforces post-discharge care plans; identifies barriers to care.
