Job summary

BAYADA Home Health Care in Jacksonville, FL is seeking a compassionate full-time Start of Care/Oasis Admissions Registered Nurse to join our team. You will visit adult and geriatric clients in Duval, Baker, Clay and Nassau Counties on a per-visit basis, administer meds, wounds, and coordinate care.

The RN must hold a FL license, have at least 1 year of clinical experience; home health experience preferred. Travel to cases is required and training is provided.

Qualifications

• Current FL Registered Nurse license.
• Minimum 1 year of recent clinical (medical/surgical) experience.
• Home care experience preferred but not required.
• Graduation from an accredited nursing program.
• Ability to travel to cases as assigned.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Follow a designated care plan in accordance with patient's needs.
• Make home visits to clients in designated geographic territories.
• Perform administration of medication, wound care, treatments, and procedures.
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager.
• Follow up with, execute, and properly document doctors' orders.
• Perform client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations in EMR on a touch pad tablet.
