Job summary

Baptist Home Health Care by BAYADA is seeking a compassionate Registered Nurse (RN) to join our team in Eastern Jacksonville, FL. This full-time role provides in-home visits to adult and geriatric clients, following individualized care plans with medication administration, wound care, and ongoing assessments.

The RN will collaborate with Clinical and Client Services Managers, document orders precisely, and contribute to care management.

Qualifications

• Current FL RN license is required.
• Minimum 1 year of recent clinical (medical/surgical) experience.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Ability to travel to assigned cases.
• Florida Background Screening Clearinghouse adherence.

Responsibilities

• Follow designated care plans tailored to patient needs.
• Make home visits to clients in designated geographic territories.
• Administer medications, perform wound care, and treatments.
• Monitor client conditions and report changes to supervisor.
• Document doctors' orders and observations accurately.
• Perform client assessments and coordinate case management.
