Job summary

Baptist Home Health Care by BAYADA in Jacksonville, FL is seeking a compassionate, dedicated RN to join our team on a full-time basis for Start of Care/Oasis Admissions. You will visit patients across Duval, Baker, Clay and Nassau counties, following care plans and coordinating with a multi-disciplinary team to deliver high-quality home health care.

The ideal candidate holds a FL RN license with at least one year of clinical experience, strong interpersonal and computer skills, and a willingness

• A current Florida RN license.
• Minimum of one year of clinical (medical/surgical) experience.
• Home health experience preferred but not required.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Ability to travel to assigned cases.
• Florida Background Screening Clearinghouse.

Responsibilities

• Follow designated care plans according to patient needs.
• Make home visits to clients in designated geography.
• Perform meds administration, wound care, treatments, and procedures.
• Monitor client conditions and report changes to supervisors.
• Follow up on doctors' orders and document them.
• Conduct client assessments as needed.
• Coordinate case management and services.
• Document observations and care activities on a tablet.

This office services our adult and geriatric clients on a per visit basis in territories throughout Duval, Baker, Clay and Nassau Counties.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the art tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in FL.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
