Job summary

Baptist Home Health Care by BAYADA in Jacksonville, FL is seeking a compassionate full-time Start of Care/Oasis Admissions Registered Nurse to join our team. The RN will visit adult and geriatric clients in Duval, Baker, Clay and Nassau Counties on a per-visit basis, delivering skilled nursing care under an established care plan.

The ideal candidate holds a Florida RN license, has at least one year of clinical experience (home health preferred), is comfortable traveling, and can document care

Qualifications

• Current FL Registered Nurse license.
• Minimum 1 year of clinical (medical/surgical) experience.
• Home care experience preferred.
• Graduation from an accredited nursing program.
• Ability to travel to cases as assigned.
• Strong interpersonal and communication skills.
• Solid computer skills; EMR experience preferred.
• Document patient care using state-of-the-art technology.

Responsibilities

• Follow designated care plans for patients.
• Make home visits to clients in designated territories.
• Administer medication, wound care, treatments, and procedures.
• Monitor client conditions and report changes.
• Follow up with doctors' orders and document accordingly.
• Perform client assessments as needed.
• Coordinate case management and services.
• Document observations and interventions on a tablet.
