About This Role

The OASIS RN Weekend position involves providing skilled nursing care to clients in their homes or community health settings. This role is ideal for nurses who are dedicated to delivering high-quality, outcome-based care.

• Schedule: Part Time, Weekend shift

What You'll Do

• Provide skilled nursing care to clients in their homes or community health settings.
• Conduct a complete nursing assessment based on referral data and clinical observation.
• Complete the OASIS assessment with accurate scoring of clinical & functional questions.
• Develop a plan of care in collaboration with clients and all disciplines.
• Provide OASIS time point assessments throughout a patient's episode.
• Initiate and maintain a client record utilizing the Agency's standard for documentation.
• Make follow-up visits to assess disease progress and evaluate response to treatment.
• Teach patients and reinforce client/caregiver education.
• Refer, collaborate, and coordinate all client care services.
• Participate in client care conferences, staff meetings, and health care programs.

Requirements

Education:

• Graduate of an Associate’s Degree or diploma program in nursing

Experience:

• One year nursing experience or one year nursing experience in a specialty

Licenses & Certifications:

• Registered nurse with a license to practice in the state of CT
