Job summary

Hartford HealthCare at Home seeks a Baylor Homecare RN to join an experienced multi-disciplinary team. This 24-hour weekend position provides skilled nursing care to patients in the community, delivering high-quality, outcome-focused services.

Licensed in CT, with at least an Associate's or diploma in nursing; BSN preferred. One year of nursing experience or home care specialty is preferred. Comprehensive benefits included."

For: either Friday, Saturday, Sunday or Saturday, Sunday, Monday 24 hour position to provide skilled services to our patients in the community.

As part of a person centered care model, this role will provide skilled nursing care to clients in home and community based settings ensuring the delivery of outcome based, high quality, cost effective care. This role will collaborate with all team members to achieve exceptional outcomes.
