Job summary

Hartford HealthCare at Home is seeking a Visiting Registered Nurse (RN) to perform OASIS assessments and coordinate care for patients in the Watertown area. This role supports Waterbury and surrounding towns with a focus on delivering patient-centered home health services.

Qualified candidates hold a CT RN license, have at least 1 year of RN experience (homecare preferred), a valid driver's license, and strong computer skills to document care using modern technology.

Qualifications

• CT RN license required.
• Minimum 1 year of RN experience; homecare experience preferred.
• Valid driver's license and reliable transportation.
• Computer proficiency required.

Responsibilities

• Perform comprehensive OASIS assessments to establish an individualized care plan.
• Facilitate communication between the patient and the interdisciplinary team.
• Develop and maintain therapeutic relationships by providing education and promoting self-advocacy and independence.
• Utilize state-of-the-art technology to document skilled services provided.

Location Detail: 680 Main Street Watertown (10326)

Shift Detail: Mon-Fri 8-4:30 cover Waterbury, surrounding towns and Naug Valley. Competent in Oasis 1 weekend/month, 1 major and 2 minor holidays/yr
