Job summary

Harmony Home Health Inc. is seeking an OASIS Coordinator (OASIS Review, Coding & QA) in a full-time hybrid role.

The coordinator will review OASIS assessments in Axxess for accuracy, assign ICD-10-CM codes, and perform QA to ensure regulatory compliance and proper reimbursement. The role requires an active RN license (or LPN/LVN as appropriate), 2+ years in home health, OASIS experience, and strong attention to detail.

Qualifications

• Active RN license or LPN/LVN as per state requirements.
• Minimum 2 years of home health clinical experience.
• Experience with OASIS assessments (OASIS-D/E) and home health coding.
• Hands-on experience with Axxess software (clinical, QA, and/or coding).
• Working knowledge of ICD-10-CM coding guidelines as applied to home health.
• Familiarity with CMS home health regulations, PDGM, and Conditions of Participation.
• Strong attention to detail and analytical skills.
• Reliable home internet/workspace setup to support hybrid work arrangement.

Responsibilities

• Review OASIS assessments in Axxess for accuracy and consistency.
• Assign and validate ICD-10-CM diagnosis codes per guidelines.
• QA review of clinical documentation for CMS compliance and agency policy.
• Identify documentation gaps and communicate with field clinicians via Axxess workflows.
• Ensure timely submission of OASIS data within CMS timeframes.
• Monitor case-mix and outcome data using Axxess reporting.
• Collaborate with clinicians, case managers, and billing staff to resolve discrepancies.
• Stay current on CMS OASIS updates and related regulations.
• Maintain QA records to support audit-readiness.
• Provide education/feedback on documentation and coding practices to staff.
