Job summary

Apploi is seeking a compassionate Social Worker to join our healthcare team. You will assess residents' social, emotional, and psychological needs, develop care plans with the interdisciplinary team, and connect residents with community resources to support well-being.

You will provide counseling, document interactions per policy, and contribute to quality improvement initiatives. A MSW is preferred, and NY licensure as LSW or LCSW is required or strongly encouraged.

Qualifications

• MSW preferred; Bachelor's degree in Social Work accepted.
• Licensed in NY as LSW or LCSW.

Responsibilities

• Assess the social, emotional, and psychological needs of residents and provide personalized support.
• Develop and implement comprehensive care plans with the interdisciplinary team.
• Facilitate connections with community resources, services, and organizations to support residents' well-being and independence.
• Provide emotional support and counseling to residents, families, and staff members.
• Document resident interactions, assessments, and treatment plans in accordance with policies and procedures.
• Participate in quality improvement initiatives and contribute to best practices in social work services.
