Job summary

Apploi is seeking a compassionate Social Worker to join our healthcare team. You will assess residents' social, emotional, and psychological needs, develop care plans with the interdisciplinary team, and connect residents with community resources to support well-being.

You will provide counseling, document interactions per policy, and contribute to quality improvement initiatives. A MSW is preferred, and NY licensure as LSW or LCSW is required or strongly encouraged.

Qualifications
