Zusammenfassung

NYC Health + Hospitals Queens Hospital Center is seeking a licensed social worker to provide psychosocial services across patient care settings. Under general supervision, you will perform assessments, develop treatment plans, and deliver individual and group counseling to support patient-centered outcomes.

The role involves multidisciplinary collaboration, outreach, documentation for mandated reporting, and support for discharge planning.

Qualifikationen

Valid LMSW license issued by the NYSED.
Valid LCSW license issued by the NYSED.

Aufgaben

Screens charts for psychosocial information and conducts psychosocial evaluations to inform treatment plans.
Provides individual and group counseling in hospital or community settings.
Collaborates with professionals from other disciplines for multidisciplinary patient care and discharge planning.
Provides outreach and referrals to governmental, public, and community agencies; assists high-risk patients.
Documents suspected child abuse or neglect for NYS Central Registry; testifies in hearings as needed.
Assists in planning post-institutional/hospital care and alternative arrangements.
Participates in staff meetings, QA/PI activities, and maintains required records.
Ensures compliance with NYC Health + Hospitals policies and external standards; maintains safety and quality standards.
May provide clinical services under supervision of a NYS LCSW, licensed psychologist, or psychiatrist.
Mentors and supports less experienced staff and may supervise students or volunteers.
