Northern Metropolitan Center in New York is seeking a dedicated Social Worker to join our care team. You will assess, develop, and implement individualized care plans addressing physical, emotional, and social needs of patients and residents.

You will provide counseling, coordinate with healthcare providers, connect families with community resources, and maintain accurate records as part of an interdisciplinary team.

Qualifications

• Minimum 0 years of experience in social work or a related field.
• Bachelor's degree in Social Work or a related field.
• Current licensure as a Social Worker (LSW, LCSW, etc.)
• Excellent communication, interpersonal, and problem-solving skills.
• Ability to work effectively in a fast-paced environment with multiple priorities.
• Full-time commitment to our team.

Responsibilities

• Conducting social histories and assessments to identify patient/resident needs and develop personalized care plans
• Providing counseling, education, and support to patients, residents, and their families
• Coordinating with healthcare providers and other professionals to ensure comprehensive care
• Facilitating connections with community resources and services
• Maintaining accurate and comprehensive records of patient/resident care
• Participating in interdisciplinary team meetings and contributing to continuous quality improvement initiatives
