JOB DESCRIPTIONPOSITION TITLE: Nutrition MonitorDEPARTMENT: Nutrition/Day Care HomesFLSA STATUS: Non-ExemptPAY GRADE: 4REPORTS TO: Program Manager – SeniorsGENERAL DESCRIPTION OF DUTIESThis candidate will serve as a resource for technical and administrative support for the Home Delivered Meal Program (MOW) and Congregate Meal Program.SPECIFIC DUTIES AND RESPONSIBILITIESESSENTIAL JOB FUNCTIONSThe list of essential functions, as outlined herein, is intended to be representative of the tasks performed within this classification. The omission of an essential function does not preclude management from assigning duties not listed herein if such functions are a logical assignment to the position.Provide Assistance to Program Manager and Nutrition Manager on a daily basisCoordinate, record and follow-up route and participant changes for all clients in the Mid-state area, Greater Hartford area, as well as those in Suffield, Newington, Windsor Locks, East Windsor, South Windsor, Stafford Springs, Somers ,Hebron, and works closely with Site Coordinators in Enfield and Hockanum.Prints daily route sheets and makes changes as necessary to comply with order of delivery and client requestsDaily production of labels for meal identification and ensure that all meals are properly labeled and in delivery sequenceInput weekly invoices into Nutrition Spend down and check for accuracy.Separate donation checks by fund, town, and place in alphabetical order by town name for Nutrition Manager to make bank deposits.Ensures all phone calls are answered from clients and home delivery drivers and document each call-in client intake area of SERV TrackerPerforms various customer service-related tasks, including receiving phone calls from various clients and/or referral sourcesPulling or adding meals on wheels when neededReview and enter new client Form 5 data into SERV Tracker to begin meal service for both MOW and congregate programsInforms Site Coordinators of participant meals changesConduct in-person intake for Mid-State clients as neededReassess meals on wheels clients as neededProvide support at congregate meal sites as neededEnters weekly meal counts from sitesPrepares monthly packages of required paperwork for congregate sitesMaintain organized records and make reports as required by Program, including weekly and monthly reportsADDITIONAL JOB FUNCTIONSPerform all other duties as assigned.MINIMUM TRAINING AND EXPERIENCEEducation: Associate degree preferred in Human ServicesMinimum Years of Experience: 1-year experience in related field preferredActive Certification in good standing: Must have a valid driver’s license and good driving record is requiredDemonstrate skills: Computer experience is needed. Candidate must also have excellent oral and written communication skills.Knowledge: Knowledgeable of MS Word and MS Excel is required.
