Job summary

Holy Cross Hospital, part of Trinity Health, seeks a nursing leader to manage patient care coordination on the unit. You will oversee staffing, care planning, discharge planning, and the implementation of nursing processes while guiding a multidisciplinary team to deliver high-quality patient care.

You will participate in staff interviews, lead daily huddles, monitor quality indicators, and ensure adherence to safety and infection prevention protocols, with opportunities for professional growth

Qualifications

• Graduate from ACEN/CCNE accredited program.
• Experience in nursing leadership on a hospital unit.
• Ability to coordinate patient care plans and discharge planning.

Responsibilities

• Coordinate patient care activities on the nursing unit.
• Coordinate multidisciplinary care including care planning and discharge planning.
• Ensure staffing coverage for unanticipated needs; assist in staff scheduling.
• Participate in interviewing prospective staff and provide input to the Director.
• Lead huddles and monitor PI indicators; ensure patient safety and staff accountability.
