Job summary

Horizon Home Care & Hospice, Inc. is seeking a Home Health Aide (CNA) to provide direct patient care in home settings across Milwaukee County and surrounding areas. You will assist with daily living activities, comfort measures, and coordination with the Case Manager.

Flexibility is key: part-time (8–36 hours/week) or on‑call pool; prior public-facing experience preferred; Wisconsin CNA certification and a valid driver’s license required; mileage is reimbursed.

Qualifications

• Wisconsin CNA certification allowing work in Federally Certified Home Health Agencies and Hospices.
• Current and valid Wisconsin Driver’s License and reliable, insured vehicle.
• Ability to work flexible hours, including part‑time (8–36 hours/week) or on‑call pool schedule.

Responsibilities

• Support patient care by reviewing care plans prior to providing care and notifying the Case Manager of any concerns or required revisions.
• Provide assistance with daily living tasks such as bathing, dressing, transfers, range-of-motion, toileting, and other activities under the supervision of the Case Manager.
• Explain the care being provided and involve patients in their care.
• Complete mobile care tasks upon arrival and departure from patients' homes, including mileage log reporting in accordance with Agency policy.
• Communicate with patients, family members, care providers, and Agency staff, listening and responding to patient needs and reporting pertinent information to the Case Manager in a timely manner.
• Perform home maintenance tasks as outlined in the plan of care, e.g., house cleaning, washing dishes, laundry, meal preparation, and transportation.

Position Overview

Work Setting

Employees will work in patients' homes across Milwaukee, Waukesha, Ozaukee, Waukesha and Washington Counties (mileage reimbursed between visits).
