Job summary

Horizon Home Care & Hospice in Milwaukee seeks a dedicated Home Health Aide to join our interdisciplinary Hospice team. You will provide essential personal care services to patients in their homes, ensuring their comfort and well-being under the supervision of a Registered Nurse.

This full-time role requires a Wisconsin CNA certification and offers competitive pay with benefits including a sign-on bonus, mileage reimbursement, and a comprehensive benefit package. Shift hours are Monday to Friday from 8:00 am to 4:30 pm.

Qualifications

• Wisconsin CNA certification allowing work in Federally Certified Home Health Agencies.
• Experience in a public-facing role is required.
• Current/Valid WI Driver's License and a reliable insured vehicle.

Responsibilities

• Support patient care by reviewing care plans and notifying Case Manager of concerns.
• Assist with daily living activities such as bathing and dressing.
• Provide emotional support by explaining care processes to patients.
• Complete mobile care tasks timely and accurately as per agency policy.
• Cooperate with patients, family members, and Agency staff regarding patient needs.

Role Summary

As part of our interdisciplinary Hospice team, our CNA’s (Home Health Aides) provide patients with care in the comfort of their own home and/or similar setting. As a Home Health Aide in Hospice you will provide direct assistance with activities of personal care, maintenance of patient health, agency facilitation of treatment for patients, and contribution to a health environment for patients. These activities are provided under the direction and supervision of a Registered Nurse or Therapist, in accordance with the physicians' orders.

Responsibilities

• Support patient care by reviewing care plan in the home prior to providing cares. Notify Case Manager with issues/concerns regarding care plan and when cares need revision.
• Provide assistance with daily living under the supervision of the Case Manager e.g., bathing, dressing, transfers, range of motion, toileting.
• Explain to patient the care being provided and involve patient in cares.
• Consistently complete mobile care upon arrival and departure from patients' home per Agency policy. Mobile Care includes tasks completed in accordance with the care plan. Complete mileage logs timely and accurately.
• Work cooperatively with patients, care providers and family members, as well as with appropriate Agency staff by listening and responding to patient needs, engaging in appropriate communication, reporting pertinent patient information to Case Manager in a timely manner.
• Regularly complete home maintenance per plan of care e.g., house cleaning, washing dishes, laundry, meal preparation, and transportation.

Qualifications

Hours & Compensation

Full‑Time, 1st shift - Monday‑Friday 8:00 am‑4:30 pm. Non‑Exempt, hourly.
