Job summary

Green Key Resources is seeking a Clinical Supervisor / RN Field Supervisor for a growing home care organization in Rockland, NY. The role blends field-based clinical visits, patient assessments, staff supervision and care coordination.

You'll conduct in-home supervisory visits, complete assessments, oversee staff performance, ensure Plan of Care implementation, and participate in on-call rotation with a stipend.

Qualifications

• Active NY RN license.
• 2–3+ years nursing experience.
• Home care experience preferred.
• Supervisory experience preferred.

Responsibilities

• Conduct in-home supervisory and assessment visits.
• Complete 6-month assessments and supervisory visits.
• Assess patient needs, home environment, safety, and Plan of Care implementation.
• Evaluate staff performance and competency.
• Identify clinical concerns and communicate findings to the care team.
• Provide coaching and clinical guidance to field staff.
• Respond to clinical questions and incidents from patients, families, and staff.
• Collaborate with intake, scheduling, and operations on case starts and care plan updates.
• Complete timely documentation in the EMR.
• Support quality improvement initiatives through chart audits and process improvement.
• Participate in an on-call rotation approximately every 4–6 weeks with stipend.

Clinical Supervisor / RN Field Supervisor – Home Care

A growing home care organization in Rockland is looking for a Clinical Nurse Supervisor. This is a great opportunity for a home care RN who enjoys a combination of field-based clinical work, patient assessment, staff supervision, and office-based care coordination.

Monday–Friday | 8:30 AM–5:00 PM

What You’ll Do:

• Conduct required in-home supervisory and assessment visits
• Complete 6-month assessments, supervisory visits, and intermittent visits
• Assess patient needs, home environment, safety, and Plan of Care implementation
• Evaluate HHA and personal care staff performance and competency
• Identify clinical concerns and care gaps and communicate findings to the care team
• Provide coaching and clinical guidance to field staff
• Respond to clinical questions and incidents from patients, families, and staff
• Collaborate with intake, scheduling, and operations on case starts, transitions, and care plan updates
• Complete timely documentation in the EMR
• Support quality improvement initiatives through chart audits and process improvement
• Participate in an on-call rotation approximately every 4–6 weeks by phone, with an additional stipend for on-call weeks

What We’re Looking For:

• 2–3+ years of nursing experience
• Home care experience strongly preferred
• Open to candidates with backgrounds in MLTC, CHHA, licensed home care, care management, UAS, or similar home care settings
• Supervisory experience strongly preferred
• Knowledge of NYSDOH home care regulations and supervisory visit requirements
• Strong assessment, communication, and coaching skills
• UAS and HHA Exchange experience preferred
• Valid NY driver’s license, reliable transportation, and auto insurance
