The Caring Connections is seeking a dedicated Home Care Nurse to provide high-quality healthcare in patients' homes in Westchester County. The RN conducts in-home assessments, develops or updates the Plan of Care, and ensures caregivers implement the plan.

You will supervise care, instruct patients and families, and perform monthly reassessments. Start-of-care assessments must occur quickly; occasional visits to Rockland County may be required; strong English and independence are essential.

Qualifications

• Active NY RN license and registration.
• Valid driver's license and reliable transportation.
• Clear English and excellent communication.

• Previous homecare or assessment nursing preferred.
• Up-to-date immunizations and PPD test.

Responsibilities

• Complete start‑of‑care assessments within 24 hours of care start.
• Lead in‑office orientations for new caregivers at the Newburg Office.
• Submit all visit documentation to the DPS within 24 hours.
• Communicate concerns or client changes to the DPS; may perform occasional Rockland County visits.

Overview

We are seeking a dedicated and compassionate Home Care Nurse to provide high-quality healthcare services to patients in their homes. The ideal candidate will possess a strong clinical background and demonstrate excellent patient assessment skills. This role is essential in ensuring that patients receive the necessary care and support in a comfortable environment, promoting their overall well‑being and recovery.

We are looking to hire a Registered Nurse to join our team, to conduct IN-HOME assessments in Westchester County.

This is a Full Time position.

Job Description

The RN will work under the direction of the DPS (director of patient services) to evaluate client’s home care needs. These visits are to assess the client’s homecare needs, create or adjust a Plan of Care, and oversee that the caregiver is implementing the plan of care appropriately.

Instruct and advise patients and/or families as well as the home health aides in their care.

Reassessments will be scheduled at the beginning of each month, according to both the nurse’s and client’s availability. An aide annual supervision list will be assigned at the beginning of the month. Start‑of‑care assessments are on a non‑predictable schedule but must be completed within 24 hours of the start of care. PRN visits may be required if there is a change of condition that requires a nurse’s follow‑up.

The nurse will conduct in‑office orientations to all new caregivers in a group setting, on a predetermined schedule, in our Newburg Office.

The nurse is responsible for submitting all documentation from visits to the DPS within 24 hours of the visit, and for communicating any concerns or client changes to the DPS. Occasional visits may be required in Rockland County when the Rockland nurses are out.
