Job summary

Arc Mercer, located in Mercer County, NJ, seeks a high‑skill, autonomous RN to lead clinical operations in residential IDD care. This role centers on proactive risk management, clinical leadership, and hands‑on nursing excellence within a supportive, mission‑driven setting.

You will oversee a caseload, conduct assessments, train staff, and coordinate hospital transitions while advancing a proactive, risk‑based clinical model that improves health outcomes and reduces hospitalizations.

Qualifications

• Active RN license (New Jersey) required.
• ADN or BSN degree required.
• 2+ years nursing experience (IDD, home health, community health, or case management preferred).
• Strong clinical assessment skills.
• Excellent communication and collaboration abilities.
• Valid driver’s license & reliable transportation.

Responsibilities

• Risk-Based Oversight — Manage a dedicated caseload across Arc Mercer residential homes, tailoring visit frequency to individual acuity.
• Clinical Assessments — Perform in-person nursing assessments, observe behavioral/physical changes, and escalate care early.
• Appointment Preparation — Strategize with staff before appointments, prep documentation, and ensure closed-loop follow-through.
• Chronic Disease Management — Lead proactive monitoring for diabetes, hypertension, respiratory conditions, seizure disorders, and more.
• Medication Safety — Own reconciliation, MAR accuracy, interaction monitoring, and medication error investigations.
• Staff Training — Coach DSPs and Residential Managers through real-time, on-site clinical education.
• Behavioral Integration — Partner with Behavioral Services to identify medical causes behind behavioral changes.
• Hospital & ER Transitions — Participate in discharge planning, secure equipment, and ensure seamless home transitions.
• Clinical Huddles — Join weekly interdisciplinary huddles to review trends, risks, and upcoming procedures.
• Documentation & Quality — Track key health metrics, appointment completion, medication errors, and readmission trends.

Compensation: $95,000 - $110,000 annually
