Job summary

Active Day in Hopkinsville, KY is seeking a Licensed Practical Nurse to provide nursing care to members according to physician orders and emergency needs, while ensuring safety and regulatory compliance. Qualifications

• LPN license and CPR certification required.
• Must be at least 21 years old.
• Prefer at least 1 year full-time equivalent experience in a health care setting.

• Able to multi-task and manage time effectively.
• Flexible schedule required.
• Physical ability to bend, lift up to 50 pounds, and stand for extended periods.
• Background check compliance and state regulatory requirements as applicable.

Responsibilities

• Provide nursing care to members according to the individual care plan and physician orders, including emergency needs.
• Observe, monitor, and document vital signs and responses to treatments and medications; report abnormalities to the Director of Nursing.
• Assist members with daily activities such as hygiene, meals, mobility, arrivals and departures, and social activities.
• Provide health education and instruction to staff, members, and family members.
• Provide emergency care as needed.
• May administer medications with appropriate certification and supervision by the Registered Nurse.
• Maintain positive, courteous, and professional communication with members, families, co-workers, referral agencies, and the community.
• Participate in staff meetings and in-service programs; collaborate with cross-functional teams to enhance member experience.
• Comply with company policies, safety, and regulatory laws and standards.
• Other duties as assigned.

Duties involve providing instruction and assistance to members, staff and family members. The Licensed Practical Nurse ensures that all work complies with company policies and procedures, safety and regulatory laws and standards.

Qualifications

• At least 21 years of age.
• At least one-year full-time equivalent experience in a health care setting preferred.
• Prior experience working seniors and/or adults with special needs preferred.

• Able to work a flexible schedule.
• Able to bend, pull, push, squat, turn, transfer, and lift 50 pounds; able to stand for an extended period while providing physical assistance to members.
