Job summary

Active Day is seeking a Licensed Practical Nurse to provide nursing care to members according to individualized plans, physician orders, and emergent needs. You will observe and document vital signs, respond to treatments, and ensure care aligns with safety and regulatory standards.

You will assist members with daily activities, educate staff and families, provide emergency care as required, and collaborate with a cross-functional team to improve member experience while complying with company

Qualifications

  • LPN License and CPR certification; malpractice insurance if applicable.
  • At least 21 years of age.
  • Prefer at least 1 year full-time equivalent healthcare experience.
  • Prior experience with seniors or adults with special needs is preferred.
  • Strong communication and interpersonal skills.
  • Ability to multi-task, manage time, and work a flexible schedule.
  • Able to lift up to 50 pounds and stand for extended periods.
  • Successful background check and applicable state regulatory requirements.

Responsibilities

  • Provide nursing care per plan of care, orders, and emergent needs.
  • Observe, monitor, and document vital signs and treatment responses.
  • Assist with daily activities: hygiene, meals, ambulation, arrivals/departures, social activities.
  • Provide health education and instruction to staff, members, and families.
  • Provide emergency care as required.
  • May administer medications with appropriate certification and supervision by RN.
  • Communicate professionally with members, families, coworkers, referral agencies.
  • Participate in staff meetings and collaborate with cross-functional teams.
  • Comply with policies, safety, and regulatory laws.
  • Other duties as assigned.

Job Summary

The Licensed Practical Nurse provides nursing care to members in accordance with physician's orders and emergent needs. Duties involve providing instruction and assistance to members, staff and family members. The Licensed Practical Nurse ensures that all work complies with company policies and procedures, safety and regulatory laws and standards.

Qualifications
  • LPN License, CPR certification, and valid malpractice insurance policy if applicable.
  • At least 21 years of age.
  • At least one-year full-time equivalent experience in a health care setting preferred.
  • Prior experience working seniors and/or adults with special needs preferred.
  • Strong communication and engaging interpersonal skills.
  • Able to multi-task and manage time effectively.
  • Able to work a flexible schedule.
  • Able to bend, pull, push, squat, turn, transfer, and lift 50 pounds; able to stand for an extended period while providing physical assistance to members.
  • Successful results of background check, and where applicable, additional state-specific regulatory requirements.
Job Responsibilities
  • Provides nursing care to members according to individual plan of care, physician's orders and emergency needs.
  • Observes, monitors and documents members' vital signs and responses to treatments and medications, reporting abnormalities to Director of Nursing.
  • Assists members as needed with daily functional activities, including but not limited to, hygiene, meals, ambulation, arrivals and departures, and social activities.
  • Provides health education and instruction to staff, members and family members.
  • Provides emergency care as required.
  • May be required to administer medications with appropriate certification and supervision by the Registered Nurse.
  • Communicate in a positive, courteous and professional manner with members and their families, co-workers, referral agencies, and the community at large.
  • Participate in staff meetings and in-service educational programs as requested; actively collaborate with cross-functional team, providing feedback and recommendations for enhancing the member experience and achieving the center's overall goals and objectives.
  • Comply with company policies and procedures, safety and regulatory laws and standards.
  • Other duties and responsibilities as changed or assigned at any time.
