Job summary

The Addison of St. Johns in Jacksonville, FL, part of Sonida Senior Living, seeks a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) to lead nursing assistants in our assisted living and memory care community.

In this role you will direct day-to-day nursing activities, document resident care, coordinate with physicians and pharmacies, and ensure medications and supplies are maintained for residents. A minimum of 1 year of LPN/LVN experience and active licensure is required.

Qualifications

• Must be licensed as an LPN/LVN and maintain current licensure.
• Minimum 1 year of LPN/LVN experience.

Responsibilities

• Direct the day-to-day nursing functions of the nursing assistants in accordance with current rules and guidelines.
• Administer medications as ordered by the physician.
• Make written and oral reports/recommendations concerning shift activities.
• Ensure adequate supply of medications, supplies and equipment; report needs to nursing supervisor.
• Place phone calls to pharmacies to order prescriptions.
• Pulls and prepares patient charts; perform general office duties as required.

LPN/LVN Responsibilities Include

• Administer all medications as ordered by the physician
• Make written and oral reports/recommendations concerning the activities of your shift as required
• Ensure adequate supply of medications, supplies and equipment is on hand to meet the nursing needs of residents. Report needs to the nursing supervisor
• Place phone calls to pharmacies to order prescriptions
• Pulls and prepares patient charts; performs other general office duties as required

Qualifications

• Minimum 1 year experience required
• Must be licensed as an LPN/LVN, and must maintain current and unencumbered licensure
