Job Overview

The Care Manager/Designated Care Manager is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living and long‑term care neighborhoods.

Responsibilities

• Review residents’ care and complete the Daily Log documenting pertinent information.
• Participate in developing and updating Individualized Service Plans (ISPs).
• Maintain a designated group of residents during the shift, inspecting their condition and reporting changes.
• Respond promptly to call bells, security system requests, and telephone calls.
• Assist residents with continence management and maintain cleanliness of suites and community restrooms.
• Help residents maintain independence, physical safety, and dignity, adhering to Resident Rights and Sunrise Principles of Service.
• Lead and assist residents with activities of daily living (ADLs) and invite them to participate in activities as indicated on the ISP (IEA).
• Engage residents in life skill and enrichment activities, including reminiscence.
• Monitor safety regulations, practice Standard Care Precautions, and participate in risk management and general safety activities.
• Maintain housekeeping and laundry responsibilities such as bed making, trash removal, laundry washing, and cleaning of common areas.
• Serve meals in the dining room, follow diet plans from ISPs, and assist with dining room set‑up and clean‑up.
• Provide room‑service delivery and practice safe food handling.
• For the Reminiscence Neighborhood Care Manager: IEA residents in life skills and enrichment activities, incorporate “Creating Pleasant Days,” and develop multi‑sensory experiences.
• Participate in team engagement activities, training, and compliance with infection control and state‑required training.
• Perform other duties as assigned.

Qualifications

• High School diploma or GED (required or preferred per state regulations).
• CPR and First Aid certification, as required by state or provincial regulations.
• Minimum age 18 years.
• Previous experience working with seniors preferred.
• Desire to serve and care for seniors.
• Ability to make choices and decisions in the resident’s best interest.

• Basic technical competence and good judgment.
