IPRO, based in Syracuse, NY, seeks a Nursing Home Surveyor / Complaint Investigator to support NYSDOH surveillance and investigations of long-term care facilities. The role covers reviewing abuse/neglect, creating reports, and SODs, with extensive travel to listed counties in Upstate NY.

Non-travel days may be in the Syracuse office or telecommuting if approved; the role requires obtaining SMQT within 12 months and maintaining professional communication with diverse stakeholders.

Qualifications

• Must be NYS Registered Dietitian (RD).
• Graduate of registered professional dietitian program; bachelor's degree preferred.
• Two years’ surveillance experience in a nursing home or two years' clinical healthcare experience.
• SMQT certification preferred and must be obtained within 12 months of employment.
• Must have a valid driver's license and be able to travel to on-site assignments.
• If remote work is approved, secure internet and private workspace for work calls.

Responsabilités

• Assist NYSDOH with surveillance and investigation activities related to nursing homes.
• Review allegations of abuse/neglect and prepare written investigations and reports.
• Prepare Statements of Deficiencies (SOD) within required timeframes.
• Participate in emergency surveys and state monitoring during Immediate Jeopardy situations.

Based in Syracuse, NY the Nursing Home Surveyor / Complaint Investigator will assist the New York State Department of Health (NYSDOH), Office of Aging and Long-Term Care, with surveillance and investigation activities related to nursing homes (long term care). These activities are mandated to evaluate facility performance and monitor the quality of care and services provided. In this role you will participate as a member of an interdisciplinary survey/investigation team to review allegations of abuse and neglect, perform surveillance activities/tasks at nursing homes (long term care)/Transitional Care Units (TCUs), and Adult Day Health Care Programs (ADHCP) to determine compliance with the requirements of participation; prepare written reports of investigative findings and prepare Statement of Deficiencies (SOD) within required time frame, using Principles of Documentation. participate in emergency surveys at Skilled Nursing Facilities (SNF’s); participate in State monitoring during Immediate Jeopardy situations; as well as other duties.

The position is majority travel and will be onsite at facilities in Broome, Cortland, Lewis, Oswego, Tompkins, Cayuga, Herkimer, Madison, Oneida, Tioga, Chenango, Jefferson, Onondaga & St. Lawrence Counties.

Non-travel days are based in the NYSDOH Syracuse office or telecommuting, if approved.

Qualifications

• Demonstrate strong interpersonal skills with the ability to facilitate positive customer satisfaction in situations that are adverse or in controversy.
• Ability to successfully work independently with minimal supervision, as well as in a team environment.
• Excellent written and verbal skills with the ability to communicate professionally with co-workers, supervisors, providers, medical and administrative personnel, and residents/patients.
• Ability to translate collected information into a clear, concise, legal, and defensible document.
• Excellent investigative and analytical skills.
• Strong computer skills with working knowledge of Microsoft office products, and the ability to learn new programs.
• Ability to obtain Surveyor Minimum Qualifications Test (SMQT) certification within 12 months of employment, required.
• If approved to work remotely, must have secure internet that meets the speed required to perform all work duties, must have workspace that provides privacy for all work-related calls, and must be able to secure all work-related items when not in use.
• Must have a valid driver’s license & the ability to travel to on-site assignments, required

Education & Experience

• NYS Registered Dietitian (RD), required.
• Graduate of a registered professional dietician program, required. A bachelor’s degree in a related discipline, preferred.
• Two (2) years of experience performing surveillance activities, preferably in a nursing home; - OR- two (2) years of clinical experience in a healthcare facility, required.
• SMQT Certification, preferred.

The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future. The hourly range for this position is $36.92 - $41.54. Actual salary range and /or hourly rate depends on a variety of factors including experience, education, credentials, location, and budget.
