Job summary

UPMC Health Plan is seeking a full-time CHC Service Coordinator (Nursing Facility) to support Community HealthChoices in Allegheny County. The role uses a community-based approach to improve health outcomes and coordinate services across care settings.

The position requires travel about half the time to nursing facilities, daytime hours, Monday through Friday, and a focus on transitions to community when appropriate while ensuring quality care and regulatory compliance.

Qualifications

• Candidate must be legally authorized to work in the United States without visa sponsorship now or in the future.

Responsibilities

• Establish and build strong relationships with internal team members and partners.
• Educate on and coordinate community resources with emphasis on medical, behavioral, and social services.
• Manage an active caseload based on state mandated ratios according to residential setting, case intensity, and acuity.
• Collect program data to track participant progress and ensure OPS reporting is completed by regulatory deadlines.
• Perform professional administrative duties involving research, analysis, and reporting.
• Prepare reports and update service plans in accordance with governing bodies.
• Ensure comprehensive assessments are completed within required time frames and assess care options.
• Lead the Person-Centered Service Planning (PCSP) process and oversee PCSP implementation.
• Assist members in obtaining LTSS and Medical Assistance services.
• Coordinate with providers and community resources to support LTSS needs.
• Inform members about LTSS options, assessments, and DHS Fair Hearing rights.
• Assist with fair hearing requests when needed and requested.
• Maintain confidentiality and adhere to HIPAA requirements.
