Qualifications

• Willing to become trained in state assessments and service plans.

Responsibilities

• Promote health and wellness through coordinated care with physicians and care providers.
• Ensure compliance with Federal and State policies governing resident care.
• Collaborate with administration for referral development and occupancy goals.
• Establish rapport with residents, families, and staff across departments.
• Conduct routine resident care rounds per individualized care plans.
• Oversee assessments and care plans at required intervals.
