Job summary

GMH Grant Memorial Hospital is seeking a Part-time Clinical Coordinator RN to coordinate nursing care for assigned patients, conducting assessments and developing plans of care. The role requires a current RN license, BLS certification within 30 days, and preferably a BSN.

One year of healthcare experience is expected, with responsibilities spanning care coordination, documentation, teaching and discharge support.

Qualifications

• Current RN license in the state where services are provided or multistate license via eNLC.
• BLS certification obtained within 30 days of hire date.
• BSN preferred.

Responsibilities

• Coordinate nursing care for assigned patients using the nursing process.
• Plan, implement and evaluate care in collaboration with patient, family and health team.
• Assess and document patient status and care outcomes; adjust plans as needed.
• Administer medications with two patient identifiers and document effectiveness.
• Support discharge planning and staff education; participate in PI and quality initiatives.
