Job summary

Infinite Medical P.C. is seeking a detail-oriented on-site Nursing Care Coordinator (RN or LPN) to join our Hollywood, FL team.

The role focuses on reviewing chart documentation for CMS compliance and supporting continuity of care for seniors in long-term care facilities. The ideal candidate holds an active RN or LPN license, has geriatric care knowledge, and can work full-time Monday through Friday, 9:00 AM to 5:30 PM, with strong documentation and cross-functional communication skills.

Qualifications

• Active RN or LPN licensure
• Geriatric care knowledge
• Full-time, on-site schedule Mon-Fri 9:00–17:30
• Experience with CMS guidelines and chart review

Responsibilities

• Review patient records and RVLs for accuracy
• Identify documentation discrepancies and propose corrections
• Coordinate with HR, Operations, and Clinical teams
• Participate in mandatory meetings and cross-training
• Maintain proficiency across EMR platforms and productivity tools
