Job summary

Fairview Health Services’ M Health Fairview University of Minnesota Medical Center - West Bank Campus in Minneapolis is hiring a CNA to join a highly collaborative post-acute care team in the Transitional Care Unit system. The unit hosts multiple diagnostics and a dedicated therapy and tech staff, with a focus on getting patients home.

You will perform routine patient care, monitor conditions, assist with admissions and discharges, and ensure timely transport of patients and supplies.

Qualifications

• Current MN CNA Registry is required.
• BLS for healthcare providers required.

Responsibilities

• Provides basic patient care and assists with daily activities.
• Performs grooming, bathing, toileting, feeding and skin care.
• Assists with specimen collection/testing and accuracy in monitoring.
• Transports patients and equipment; notes and reports patient changes to RN.
• Assists with admissions, transfers and discharges; orients patient/family.

Clinical trials advance our clinical services, bringing the very newest ideas in patient care. We are conveniently located on the West Bank of the Mississippi River near downtown Minneapolis.

• Transitional Care Unit
• 28 beds
• Serves multiple diagnoses including neuro, ortho, oncology, amputee, spinal cord injury, transplant, etc.
• All adult patients
• Post-Acute - PT, OT, SLP.
• Approximately 50 therapy and tech staff at the 2 post-acute units
• Highly Collaborative between all fields along with MD/Physiatrist, rehab nursing, social work, dietary, etc. with primary goal to get patients home.

This position will be working 56 hours every two weeks on the day/evening shift, 8 hour shifts, and every other weekend. The position is eligible for benefits.

Responsibilities

• Provides basic patient care by performing routine duties to include: Partners with patient care giver in care/decision making.
• Personal grooming, bathing, toileting
• Feeding
• Skin care needs (clean and sterile dressing changes), applying ointments/creams
• Oral hygiene
• Specimen collection/testing
• Position/ambulating
• Changing linens
• Promptly answers call lights.
• Assists nurse in monitoring patient condition.
• Accurately takes and records weights, input/output, blood pressure, temperature, pulse rates, respiration, oxygen saturation.
• Places patient on/off monitors or equipment.
• Reports unusual findings to the RN in a timely manner.
• Assists with admissions, transfers, and discharge of patients.
• Orients patient/family to environment.
• Assures the patient’s belongings are accounted for, documented upon admission, transfer and discharge.
• Transports patients, equipment, specimen and supplies.
• Assures patients get to appointment on time.
• Uses transfer belt when assisting patients with transfers and/or ambulation.
• Cleans sets up, and maintains rooms and equipment.
• Orders and stocks supplies.
• Correctly documents tasks performed on appropriate form.

Required

• Current on the MN CNA Registry
• BLS (basic life support) for healthcare providers

Preferred

• Experience in a hospital setting
