Job summary

Responsibilities

  • Obtain vital signs: temp, pulse, resp, BP, pain level
  • Document abnormal values and report to RN per policy
  • Assist with patient positioning, ambulation, and transport
  • Assist with feeding, hygiene, and daily care per care plan
  • Maintain safe patient environment and observe isolation procedures
  • Complete shift reports and accurate charting; communicate changes to RN

Job description

Required: High school diploma, GED or higher level degree if hired after June 6, 2022

Licensure/Certification:

Required:

  • 2016; in lieu of CNA, one (1) complete semester of education in an accredited associate’s degree in nursing (ADN or ASN) or Bachelor of Science in Nursing (BSN) program or completion of a Licensed Vocational Nurse (LVN) program; American Heart Association BLS

California Certified Nurse Assistant (CNA) if hired after April 10,

Experience:

Job Objective:

Performs Nursing Assistant skills under the direction of clinical director, clinical coordinator or registered nurse (RN).

  • Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
  • Obtains vital signs: temperature, pulse, respirations, blood pressure and pain level.
  • Documents and communicates abnormal values and changes to RN per policy; communicates observations of patient condition changes to RN.
  • Performs AM and HS hygiene per standard.
  • Positions and ambulates patients as per care plan; assists RN with re-positioning, ambulating, transporting, and other care as directed.
  • Documents Intake, Output and daily weights; communicates information to RN; completes documentation accurately as per policy.
  • Assists with set up and/or feedings of patients.
  • Communicates with RN or appropriate team member prior to leaving unit for any reasons.
  • Assists with equipment storage, retrieval and procedure cart restocking as directed.
  • Observes all isolation procedures and regulations.
  • Provides safe patient environment at all times; stays with patient who may be in danger of falling or hurting themselves; correctly identifies patient by use of armband; ensures call bell is within reach; application of restraints.
  • Adheres to guidelines for change of shift report from assigned RN.
  • Performs patient rounds at least every hour.
  • Maintains general tidiness of patient room.
  • Completes continuing education classes as assigned.
  • Performs duties assigned by and under the supervision of the RN.
  • Performs other duties as assigned.
  • Ability to operate general office equipment (i.e., computer, printer, scanner, telephone, etc.)

  • Ability to prioritize tasks and manage time efficiently to meet deadlines
