Job summary

Aventura at Prospect Park is seeking a knowledgeable Nurse Assessment Coordinator to coordinate the MDS process and ensure accurate, timely, and compliant documentation supporting quality resident care.

The ideal candidate will hold an RN or LPN license with at least 1 year of MDS/RNAC experience in a skilled nursing or long-term care setting, and possess strong knowledge of MDS 3.0 and CMS regulations. Strong communication and attention to detail are essential.

Responsibilities

• Coordinate and complete the MDS assessment process per regulations and facility requirements.
• Ensure MDS assessments are accurate, complete, and timely.
• Collaborate with nursing, therapy, dietary, social services, and other teams.
• Participate in care plan meetings and reflect assessments in plans.
• Review documentation to identify changes in resident conditions.
• Monitor and track MDS schedules and documentation.
• Maintain compliance with federal/state MDS regulations.
• Communicate assessment findings to the interdisciplinary team.
• Address documentation gaps and support QA initiatives.
• Maintain HIPAA confidentiality and education staff on MDS processes.

NOW HIRING: Nurse Assessment Coordinator (MDS/RNAC)

Aventura at Prospect Park is seeking a knowledgeable, detail-oriented, and compassionate Nurse Assessment Coordinator to join our team. The ideal candidate will play an essential role in coordinating the Resident Assessment process and ensuring accurate, timely, and compliant documentation that supports quality resident care.

RESPONSIBILITIES

• Coordinate and complete the MDS assessment process in accordance with applicable regulations and facility requirements.




• Ensure MDS assessments are accurate, complete, and submitted within required timeframes.




• Coordinate with nursing, therapy, dietary, social services, and other interdisciplinary team members.




• Participate in interdisciplinary care plan meetings and ensure resident care plans reflect current assessments.




• Review clinical documentation to identify changes in residents' conditions and ensure appropriate interventions are documented.




• Monitor and track MDS schedules, assessments, and supporting documentation.




• Maintain compliance with federal and state regulations related to MDS, care planning, and resident assessments.




• Communicate assessment findings and resident needs with the interdisciplinary team.




• Assist with identifying and addressing documentation gaps.




• Support quality assurance and performance improvement initiatives.




• Maintain confidentiality of resident information and comply with HIPAA requirements.




• Provide education and guidance to staff regarding MDS processes and documentation as needed.
