Job summary

The Research Foundation for The State University of New York at Stony Brook is seeking a Nursing Administrator to lead clinical operations, care coordination, and program development at the Westbury and Commack clinics.

This role blends direct patient care with program leadership, mentoring LPNs and Medical Assistants, while ensuring adherence to standards and regulatory requirements. Travel between sites and day shifts are expected.

Qualifications

• Bachelor's degree in Nursing is required.
• Current New York State Registered Nurse license.
• Three years of full-time RN experience in clinical, hospital or ambulatory settings.
• Administrative experience in a healthcare setting.

Responsibilities

• Deliver comprehensive patient care and care coordination for individuals with complex and chronic health conditions, ensuring continuity across clinical services.
• Educate patients on diagnoses, treatment plans, cancer screenings, and program benefits to promote adherence and informed decision-making.
• Perform clinical duties including phlebotomy, spirometry, injections, vital signs, and cognitive assessments.
• Serve as a primary point of contact for patient inquiries via phone and patient portal; triage concerns and coordinate timely follow-up care.
• Conduct patient assessments to evaluate acuity, barriers to care, and eligibility for case management services.
• Collaborate with interdisciplinary teams to ensure coordinated, patient-centered care and assist with referrals and pre-authorizations.
