Job summary

Cantex Continuing Care Network in Lufkin, TX seeks a Nurse Manager in a skilled nursing setting to oversee patient care and ensure compliance with physician orders and standards of practice.

The role requires LVN/LPN licensure with supervisory experience; strong English communication; day shift; full-time on-site. You will lead nursing staff, monitor clinical documentation, and collaborate with the interdisciplinary team to deliver quality resident care.

Qualifications

• Graduation from an accredited Nursing School and a current and valid LVN/LPN license in the applicable State.
• Minimum of one-year supervisory experience in a long-term or health care setting is preferred.
• Ability to read, write, and speak the English language.

Responsibilities

• Make rounds on all patients at a minimum of every 4 hours and communicate findings to the charge nurse.
• Review the 24-hour report book for information on the last 24 hours and assess any changes in condition.
• Review incident reports and ensure proper documentation of nursing assessment and notification.
• Review the medical record on discharge for accuracy and follow-up care planning as needed.
• Attend weekly care plan meetings with the Interdisciplinary team and responsible party.
