The Terraces of Phoenix, a HumanGood life plan community, is seeking a Nurse Supervisor LPN to join its Wellness Team. You will oversee licensed nursing staff, ensure a safe environment, and coordinate assignments with care partners to maintain high resident satisfaction.

Responsibilities include assessing resident needs, guiding nursing care decisions, and ensuring regulatory compliance while supporting staff development and on‑call rotations.

Qualifications

• Current LPN/LVN licensed in the applying state.
• Current CPR certification.
• Prior nursing experience in senior care, skilled/post-acute setting preferred.
• PCC charting experience preferred.

Responsibilities

• Assess residents' needs at move‑in and monitor their condition over time.
• Coordinate nursing care with residents and involve them in decisions.
• Ensure staff perform duties and respond to alerts promptly and appropriately.

The Terraces of Phoenix, a distinguished HumanGood life plan community, is hiring a Nurse Supervisor LPN (LPN/LVN) for its Wellness Team.

Our Licensed Practical Nurses (LPN/LVN) are responsible for the nursing needs of residents, while actively incorporating our “Philosophy for person-directed Care.” You will ensure a safe and healthy environment for residents, staff, and visitors to participate in the resident’s daily life rhythm. You would help develop assignments in coordination with the care partners and keep each team member accountable for resident care and satisfaction.

** Full-time ** Mon. through Fri. ** 8:30 AM - 4:30 PM, including on call and holiday rotation ** $34.00 to $45.00/hr. (DOE)

** A day in the life may include, but not necessarily limited to:

• Assessing resident’s needs at the time of move in and continually evaluates residents’ condition. Promotes the highest functional level of independence possible keeping in compliance with state and federal regulations, and the community’s safety procedures
• Facilitating nursing care provided to all residents and includes residents in decision making processes
• Ensuring the nursing staff is carrying out their respective duties toward resident care. This includes monitoring and responding to arial emergency call alerts and other resident and family care needs are met appropriately and in a timely manner

To be successful in the role, you would have:

• Current LPN/LVN in the state for which applying
• Current CPR certification
• Prior nursing experience in senior care, skilled nursing, post-acute or sub-acute care preferred
• PCC charting experience preferred
