Job summary

The Terraces of Phoenix, a HumanGood community, is hiring a Nurse Supervisor LPN to lead the Wellness Team. You will oversee nursing care for residents, coordinate assignments with care partners, and ensure safety and compliance in a person-centered care environment.

This full-time role offers a day shift (Mon-Fri 8:30 AM - 4:30 PM) with on-call and holiday rotation, and a compensation range of $34.00–$45.00 per hour (DOE). Join HumanGood to support seniors and lead a compassionate team.

Qualifications

• Current LPN/LVN license in the state applying
• Current CPR certification
• Prior nursing experience in senior care, skilled nursing, post-acute or sub-acute care preferred
• PCC charting experience preferred

Responsibilities

• Assess and coordinate resident care daily with care partners to maintain safety and quality of life
• Ensure nursing staff perform duties related to resident care and respond promptly to alerts and requests
• Develop and assign care activities in collaboration with the care team while ensuring regulatory compliance

The Terraces of Phoenix, a distinguished HumanGood life plan community, is hiring a Nurse Supervisor LPN (LPN/LVN) for its Wellness Team.

Our Licensed Practical Nurses (LPN/LVN) are responsible for the nursing needs of residents, while actively incorporating our “Philosophy for person-directed Care.” You will ensure a safe and healthy environment for residents, staff, and visitors to participate in the resident’s daily life rhythm. You would help develop assignments in coordination with the care partners and keep each team member accountable for resident care and satisfaction.

** Full-time ** Mon. through Fri. ** 8:30 AM - 4:30 PM, including on call and holiday rotation ** $34.00 to $45.00/hr. (DOE)

A day in the life may include, but not necessarily limited to:

• Assessing resident’s needs at the time of move in and continually evaluates residents’ condition. Promotes the highest functional level of independence possible keeping in compliance with state and federal regulations, and the community’s safety procedures
• Facilitating nursing care provided to all residents and includes residents in decision making processes
• Ensuring the nursing staff is carrying out their respective duties toward resident care. This includes monitoring and responding to arial emergency call alerts and other resident and family care needs are met appropriately and in a timely manner

To be successful in the role, you would have:

• Current LPN/LVN in the state for which applying
• Current CPR certification
• Prior nursing experience in senior care, skilled nursing, post-acute or sub-acute care preferred
• PCC charting experience preferred
