Job summary

Cassia Community is hiring a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join the On-Call team at Haven Homes in Plymouth, MN. You will provide compassionate care, administer medications, and collaborate with a dedicated team to support older adults in a trusted, faith-based nonprofit setting.

Wages range from $36.00 to $44.50 per hour for RNs and $28.90 to $35.50 per hour for LPNs, with a $2.00 night differential. Minnesota licensure is required; new grads are welcome.

Qualifications

• Must hold a current Minnesota RN or LPN license and be in good standing (New Grads Welcome!).

Responsibilities

• Assesses, plans, implements and evaluates care plans for residents.
• Administers medications and/or treatments per physician orders and monitors response.
• Documents care using computerized medication administration and charting software.
• Notifies Clinical Manager and family of significant changes in resident condition.
• Performs additional duties as required.

Wage:

Registered Nurse $36.00 - $44.50 / hour depending on experience
Licensed Practical Nurse $28.90 - $35.50 / hour, depending on experience

Shift Differential:

Nurse Responsibilities:

• Assesses, plans, implements and evaluates care plans for residents.
• Recognizes and manages common geriatric syndromes common to aging adults: cardiovascular, respiratory, urinary, neurological, sensory and pain problems.
• Displays a courteous attitude and respect for all residents, families, and staff.
• Administers medications and/or treatments in accordance with physician's orders and monitors for effectiveness/response.
• Manages medications, supplies, and equipment including ordering, receiving, storing, and disposing of all items in accordance with policies and procedures.
• Documents care using computerized medication administration and charting software.
• Notifies Clinical Manager, attending physicians, and family members of significant changes in a resident's condition, and takes follow up action as necessary.
• Performs additional duties as required.

Nurse Qualifications:

• Must have a current MinnesotaRN or LPN license and be in good standing(New Grads Welcome!)
• Self-starter with a desire to be a part of a team.
• Excellent communication and organizational skills.
