Broad Reach Healthcare is looking for a dedicated and compassionate nurse (RN or LPN) to join our team at Liberty Commons Rehabilitation & Skilled Care Center in Chatham, MA. Qualifications

• Active Massachusetts RN or LPN license (required).
• Strong clinical assessment and documentation skills.
• Compassionate, dependable, and resident-focused.

Responsibilities

• Provide daily nursing care and medication administration according to each resident’s care plan.
• Observe, record, and report changes in resident conditions to the clinical team.
• Supervise and support CNAs and other direct care staff.
• Ensure a safe, comfortable, and respectful environment for residents.
• Collaborate with interdisciplinary team members to promote resident wellness.
• Follow all facility, state, and federal regulations for long-term care.

Shift: 7AM - 3PM or 3PM - 11PM
Pay: $31.00 – $47.50 per hour (based on experience)
Sign-On Bonus: $12,000.00 for Full-Time (pro-rated for part-time)
Location: Broad Reach Healthcare – Liberty Commons Rehabilitation & Skilled Care Center, Chatham, Cape Cod, MA
