Job summary

Broad Reach Healthcare is seeking a dedicated Nurse (RN or LPN) to provide high-quality care at Liberty Commons Rehabilitation & Skilled Care Center in Chatham, MA. You will run daily nursing care and medication administration per care plans, while building meaningful relationships with residents and families.

The role offers 7AM–3PM or 3PM–11PM shifts, a $12,000 sign-on bonus for full-time, and comprehensive benefits. Join a supportive team committed to dignity and comfort for all residents.

Qualifications

• Active Massachusetts RN or LPN license (required).
• Strong clinical assessment and documentation skills.

Responsibilities

• Provide daily nursing care and medication administration according to each resident’s care plan.
• Observe, record, and report changes in resident conditions to the clinical team.
• Supervise and support CNAs and other direct care staff.
• Ensure a safe, comfortable, and respectful environment for residents.
• Collaborate with interdisciplinary team members to promote resident wellness.
• Follow all facility, state, and federal regulations for long-term care.
