Job summary

Broad Reach Healthcare is seeking a dedicated Nurse (RN or LPN) to join our team at Liberty Commons Rehabilitation & Skilled Care Center in Chatham, MA. You will provide high-quality nursing care, administer medications, and build meaningful relationships with residents and families.

The role offers competitive hourly pay ($31.00–$47.50) with a $12,000 sign-on bonus for full-time, evening shift differential, overtime opportunities, and comprehensive benefits including medical, dental, and vision

Qualifications

• Active Massachusetts RN or LPN license (required).
• Strong clinical assessment and documentation skills.

Responsibilities

• Provide daily nursing care and medication administration per care plan.
• Observe, record, and report changes in resident conditions to the clinical team.

• Collaborate with interdisciplinary team to promote resident wellness.
• Follow facility, state, and federal regulations for long-term care.
