Job summary

Village Green Health & Rehabilitation is seeking a Staff Nurse (RN or LPN) to join a dedicated team caring for residents across short-term rehab and long-term care settings. The role emphasizes compassionate, patient-centered nursing care with supervision of CNAs and collaboration with the nursing director.

Qualifications

• Unrestricted RN or LPN license required.
• Experience in short-term rehab or long-term care preferred.

Responsibilities

• Supervise care and services provided by CNAs and other staff.
• Coordinate nursing care on the unit ensuring staff competencies.
• Provide safe and accurate medication-related interventions.
• Transcribe and coordinate physician orders accurately.
• Perform nursing assessments and promptly notify physicians of status changes.
• Develop and implement a care plan based on assessments.
• Select and institute nursing interventions to stabilize residents and prevent complications.
• Contribute to resident assessments (MDS/CAA) and care plan development.

Applicant must have unrestricted RN or LPN license.

Under the direction and supervision of the Director of Nursing, the Registered Nurse (RN) or Licensed Practical Nurse (LPN) will plan, coordinate, provide and manage nursing care, services, and health education to our residents.

These positions require patience, compassion, and a desire to care for the residents in a gentle and empathetic manner. Some responsibilities of the RN or LPN may include:

• RN/LPN will supervise the care/services provided by CNAs and other staff.

• RN/LPN will coordinate the nursing care of residents on the unit, assuring all assigned staff are capable of providing appropriate care to them.

• Provide safe and accurate medication related interventions to residents.

• RN/LPN will accurately transcribe and coordinate the implementation of physician orders.

• RN/LPN will perform nursing assessments regarding the health status of residents. Notify the physician of changes in status and promptly implement and document new orders.

• RN/LPN will develop a plan of care based on assessment, implementing nursing care.

• Select and institute appropriate nursing interventions that might be required to stabilize a resident's condition and /or prevent complications.

Contribute to the resident's assessment (MDS/CAA's) and the development of a care plan.
