Job summary

Landwell Health is seeking a Nurse Practitioner or Physician Assistant to specialize in wound care in south central Pennsylvania. You will manage wound care in skilled nursing and long-term care facilities with a Monday–Friday schedule and no nights or weekends, expanding your expertise with specialized training and opportunities toward certification.

The role covers Franklin, York, and Cumberland Counties, with travel between partner facilities and a defined territory.

Qualifications

• Active Pennsylvania NP or PA license.
• 2+ years of clinical experience preferred.
• Experience in SNF/long-term care/rehabilitation/assisted living/urgent care/geriatrics preferred.
• Wound care experience is a plus, but not required.
• DEA registration or ability to obtain.

Responsibilities

• Perform comprehensive wound assessments.
• Develop individualized treatment plans.
• Provide bedside wound care.
• Perform wound care procedures within scope of practice.
• Monitor healing and adjust treatment plans.
• Educate patients and facility staff on wound treatment and prevention.
• Collaborate with physicians, nurses, facility leadership, patients, and families.
• Maintain timely clinical documentation.
• Travel between assigned partner facilities.

What You'll Do

You'll provide specialized wound care to patients primarily in skilled nursing, long-term care, rehabilitation, and assisted living facilities throughout your assigned territory.

You'll independently manage your patients while working closely with facility clinical teams, physicians, patients, and families.

What We're Looking For

• Active Pennsylvania Nurse Practitioner or Physician Assistant license
• Ideally 2+ years of clinical experience
• Experience in SNF, long-term care, rehabilitation, assisted living, urgent care, geriatrics, or post-acute care preferred
• Wound care experience is a plus, but not required
• DEA registration or ability to obtain
• Comfortable working independently and traveling between assigned facilities
• Strong communication and clinical decision-making skills

Territory

This is a field-based position covering Franklin, York, and Cumberland Counties in Pennsylvania.

Providers generally travel within an assigned territory of approximately a 55-mile radius, with mileage and eligible tolls reimbursed.
