Nurse Practitioner with Palliative Care Experience

Full-time position.

Responsibilities

• Provide consultation in palliative care, symptom management, and supportive care to meet the needs of patients and families, as requested by the referring provider and primary team.
• Deliver direct patient care through consultation.
• Educate and support staff.
• Facilitate clarification of patient and family goals of care and access to appropriate support models of care (e.g., hospice, rehabilitation, ongoing aggressive treatment).
• Assist with the diagnosis, treatment, and management of acute and chronic health conditions, especially symptoms such as pain and dyspnea related to those conditions.

Qualifications

• Experienced Advanced Practitioner.
• Experience in palliative care.
