Nurse Practitioner – Palliative Care

We are looking for a Nurse Practitioner to provide thoughtful, comprehensive palliative care under the supervision of a physician. Your role will involve diagnosing, treating, and caring for patients while supporting families throughout their health journey with compassion and skill.
Responsibilities
- Diagnose and treat patients in hospice and palliative care settings.
- Provide ongoing medical care, symptom management, and support to patients and families.
- Coordinate care with multidisciplinary teams and oversee treatment plans.
- Assist families with decision‑making and emotional support.
- Maintain accurate patient records and documentation.
Qualifications
- Education: Graduate of an accredited masters, post‑masters, or doctoral nursing program with formal preparation as a Nurse Practitioner.
- Licensure: Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner. Current prescriptive authority with full DEA privileges. Must achieve and maintain national certification in the Nurse Practitioner specialty.
- Experience: 2 years within the last 5 years in chronic disease management/long‑term care. Hospice and/or palliative background preferred.
- Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80–100 lbs.
