Job summary

Circadia Health is seeking an on-site Nurse Practitioner to join our partner facility in West Hills, CA. You will deliver direct patient care, leverage AI-driven insights at the bedside, and coach staff to integrate Circadia workflows into care delivery.

The role requires Monday–Friday on-site presence, strong clinical judgment, and the ability to train teams while improving patient outcomes. English proficiency is required.

Qualifications

• CA NP licensed and eligible for Medicare participation.
• 2+ years NP experience with complex adult populations.
• Able to practice independently in a startup environment.
• Valid CA driver's license; comfortable local travel.
• English language proficiency required.

Responsibilities

• Direct patient care: assessments, plans of care, consent, coordination with providers.
• Tech→ bedside integration: train staff and standardize Circadia workflows.
• Interpret trends and prioritize high-risk patients for timely interventions.
• Complete documentation within the same day.
• High-volume rounding across designated facilities (30+/day as appropriate).
• Provide quality feedback to improve workflows and tech integration.
• Mentor onboarding and collaborate with clinical ops/product teams.

As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands‑on training, live demos, and shadowed assessments.

This is a full-time, embedded NP role requiring Monday–Friday, on-site presence at our partner facility, in West Hills, CA.

Responsibilities

• Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend‑based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day.
• High‑volume rounding: Efficiently complete visits across designated SNFs (typ. 30+/day where appropriate), focusing on new admits, facility‑flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting.
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.

Performance Metrics

• Visits: Target 30 patient visits/day
• Engagement: ≥80% Time to Acknowledgement Rate: <24h
• Documentation: Complete all notes using AI‑powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Qualifications

• Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid CA driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required.

Technical & Language Skills

• Tech‑savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.
