Job summary

Circadia Health is hiring a Nurse Practitioner for an on-site, full-time role in Pasadena, TX. You will deliver direct patient care to residents, train staff to integrate Circadia into clinical workflows, and translate trend data into timely interventions.

You will interpret health signals, document promptly, and work with facility teams to reduce readmissions. Candidates must be TX-licensed, ANCC-certified, with 2+ years of practice and a willingness to travel locally.

Qualifications

• Certified and licensed Nurse Practitioner in TX; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; provider bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid CA driver’s license; comfortable traveling locally.
• Thrives in a dynamic startup environment and adapts to change.
• English required.

Responsibilities

• Direct patient care: conduct initial and follow-up assessments for flagged residents and adjust plans of care.
• Tech-to-bedside integration: train staff and standardize Circadia use in clinical workflows.
• Data into action: interpret trend-based insights and drive timely interventions to prevent decline.

• High-volume rounding: 30+ visits/day across SNFs as appropriate.
• Quality & product feedback: surface field insights to improve workflows and tech integration.
• Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams.

Transform bedside care with AI. As Circadia's on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions - reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments.

This is a full-time, embedded NP role requiring Monday–Friday, on-site presence at our partner facility in Pasadena, TX.

• Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech to bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High-volume rounding: Efficiently complete visits across designated SNFs (typ. 30+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams

• Visits: Target 30 patient visits/day
• Engagement: >=80% Time to Acknowledgement Rate: <24h
• Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.
