Job summary

Circadia Health in California is hiring a full-time on-site Nurse Practitioner to support our Circadia Intelligence Platform across partner facilities in San Jose. You will conduct initial and follow-up assessments, initiate care plans, and document within AI-enabled workflows.

You will train care teams to integrate predictive health signals into routine practice, analyze trend data to prioritize high-risk residents, and ensure timely interventions while maintaining timely charting.

Qualifications

• Certified and licensed CA NP.
• 2+ years NP experience with complex adults.
• Eligible for Medicare participation.
• Valid CA driver’s license; comfortable traveling locally.
• On-site Monday–Friday; facility-based role.
• English required; Spanish preferred.

Responsibilities

• Direct patient care: conduct initial and follow-up assessments; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: train and coach staff, standardize Circadia usage in workflows.
• Data into action: interpret trend insights, prioritize high-risk patients, drive timely interventions.
• Close the loop: complete documentation within the same day.
• High‑volume rounding: perform visits at SNFs, focusing on new admits and Circadia alerts.
• Quality & product feedback: surface field insights to improve workflows and tech integration.
• Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams.

As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands‑on training, live demos, and shadowed assessments.

This is a full-time, embedded NP role requiring Monday–Friday, on‑site presence at our partner facility in San Jose, CA.

• Direct patient care: Conduct initial and follow‑up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend‑based insights, prioritize high‑risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High‑volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility‑flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams

• Visits: Target 30 patient visits/day
• Engagement: ≥80% Time to Acknowledgement Rate: <24h
• Documentation: Complete all notes using AI‑powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

• Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid CA driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

• Tech‑savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.
