Job summary

Circadia Health is seeking a full-time, embedded Nurse Practitioner to provide direct patient care at our partner facility in Stoney Point, CA. You will lead bedside integration of Circadia’s AI-driven insights into clinical workflows and coach facility teams to turn data into timely actions.

You must be a Certified Nurse Practitioner with a CA license and have at least 2 years of NP experience with complex adults. English is required, Spanish is a plus, and travel locally is expected.

Qualifications

• Must hold a CA NP license and be certified as a Nurse Practitioner.
• Minimum 2+ years NP experience with complex adult populations.
• Ability to practice independently within a clinical setting.
• Proficient in English; Spanish preferred but not required.
• Experience with EMRs and modern digital health tools is a plus.

Responsibilities

• Direct patient care with initial and follow-up assessments.
• Train staff for bedside integration of Circadia into clinical workflows.
• Interpret trend-based insights and prioritize high-risk patients for timely interventions.
• Complete documentation on the same day of each encounter.
• Conduct high-volume rounds (~25+ visits daily when applicable).
• Provide quality feedback to improve workflows and product integration.
• Mentor NP onboarding and collaborate with clinical ops/product teams.

Transform bedside care with AI. As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments.

This is a full-time, embedded NP role requiring Monday–Friday, on-site presence at our partner facility, in Stoney Point, CA.

• Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High-volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams

• Visits: Target 30 patient visits/day
• Engagement: ≥80% Time to Acknowledgement Rate: <24h
• Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

• Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid CA driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

• Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.
